This role is ideal for a nurse with at least 5 years experience in community nursing, aged care, palliative care, or chronic disease management who enjoys working autonomously while remaining connected to a supportive multidisciplinary team.
About the Role
You will provide high-quality nursing care to clients in their homes, supporting them to remain safe, comfortable, and independent within their community. The role involves working closely with all ages of clients, families, GPs, allied health professionals, and other members of our nursing team.
Key Responsibilities
· Deliver comprehensive nursing care within clients' homes.
· Complete clinical assessments and care planning.
· Monitor and manage complex health conditions.
· Support clients receiving palliative and aged care services.
· Communicate effectively with clients, families, and healthcare providers.
· Maintain accurate and timely clinical documentation.
· Participate in team meetings and collaborative care planning.

About You
To be successful in this role, you will have:
· Current AHPRA registration as a Registered Nurse.
· Current Blue and Yellow cards.
· Previous experience in community nursing, aged care, palliative care, or a related field (highly desirable).
.Understanding of aged care and/or NDIS would be preferrable.
· Excellent communication and interpersonal skills.
· Advanced computer and documentation skills.
· Strong clinical assessment and decision-making abilities.
· The ability to work independently and manage a flexible daily schedule.
· Willingness to travel between client locations throughout the region.
· A current driver's licence.
· Availability to work a minimum of three days per week.
Working Arrangements
· Weekday shifts initially.
· Future availability for weekend work required.
· No night shifts.
· Flexible part-time arrangement (minimum three days per week).

Pay: $66,678.80 – $110,407.72 per year
