About the Role
This is a rewarding part-time temporary opportunity until 30th June 2027 (60.8 hours per fortnight) based in Wangaratta, supporting carers by building trusted relationships, understanding their individual needs, and developing tailored support plans. Using an inclusive, person-centred and trauma-informed approach, you will provide information, referrals, coordinate services, and ensure the smooth delivery of the Support for Carers Program.
Working closely with the Mental Health and Wellbeing Connect Hume team, Carer Support Workers and local service providers, you will help carers access the right supports at the right time. You will create a welcoming, inclusive environment and collaborate with community agencies to achieve positive outcomes for carers and their families.
This role is classified under the Community Health Centre Stand Alone Services Social and Community Service Employees Multi Enterprise Agreement 2022 level 4 ($46.70 - $50.25 + superannuation)
Skills & Experience
To thrive in this role, you'll bring:
- Tertiary qualifications in Social Work, Social Welfare, Community Services, Youth Work, Disability, Mental Health or relevant equivalent studies (or actively working towards same)
- Skills and experience in using a strengths-based, person-centred approach to work sensitively and collaboratively with carers from diverse backgrounds, including the ability to engage people who do not self-identify as carers
- Skills and experience in intake, needs assessment and care planning. Experience in these functions with carers or with people from diverse backgrounds will be highly regarded.
