About the role:
Enhancing end of Life funding is to ensure people across NSW have equitable access to high quality end of life and palliative care. The Lead Pharmacist - Palliative care will have responsibility for providing clinical care and care coordination to people across the MLHD who have end of life and palliative care needs.
As the Pharmacist, you will:
Assess, plan, organise, deliver, evaluate, and report on the provision of high-quality end of life and palliative care pharmacy service
Assist with triage of patients to the appropriate community or outpatient pharmacy services and ensure patients have access to required medications.
Undertake clinical reviews across clinic, community and virtual care settings, either alone or in conjunction with a multidisciplinary team.
Provide clinical supervision and support to Level 1 and 2 health professionals, technical and support staff
