What you'll be doing
The CNC2 provides an advanced level nursing services as an expert in the speciality field, providing evidence-based advice and facilitating linkages and relationships with nurses, allied health and medical officers to support patient management.
The CNC2 provides expert clinical coordination, leadership and consultancy and services for people living with Parkinson’s disease (PD) to enhance quality of life and improve health outcomes
About You
You are a highly experienced and dedicated registered nurse with more than five years of post-registration experience and advanced postgraduate qualifications, including extensive experience within your specialty area. You possess comprehensive knowledge and expertise in the assessment, management and support of people living with Parkinson’s disease and other neurological conditions across both hospital and community settings. You work confidently both independently and as part of a multidisciplinary team, building strong collaborative relationships with clients, carers and healthcare providers to deliver coordinated, person-centred care.
You demonstrate exceptional organisational skills and the ability to effectively prioritise competing demands while consistently meeting key performance indicators and service objectives. Your advanced communication and interpersonal skills enable you to lead practice development initiatives, implement evidence-based change and foster positive outcomes across multidisciplinary teams. As a clinical leader, you provide strong governance, mentorship and education to colleagues, clients and service providers, supporting continuous learning and quality improvement. You are committed to applying current research and evidence to clinical practice, ensuring the delivery of high-quality, contemporary patient care. With a current NSW driver’s licence and a willingness to travel, you are flexible and responsive in meeting the needs of clients and services across a range of settings.
