We're excited to welcome an Advanced Health Worker to our Preventative Health Pathways team.
The Aboriginal and Torres Strait Islander Health Directorate works across all facilities and provides high quality care and support to Aboriginal and Torres Strait Islander patients and their families, as well as cultural education and training for Sunshine Coast Health staff.

Key Responsibilities include:
Deliver a range of evidence-based interventions in a culturally safe, clinically competent, professional and effective manner including the provision of co-case management, care coordination, intake assessments, cultural aspects of assessment and treatment plans, direct therapeutic interventions, referrals and cultural support.
Undertake health programs and community care duties that may include but are not limited to: Women's and Men's health, antenatal, child and youth health, older person's health care, nutrition, substance abuse, sexual abuse, mental health, clinics, health promotion and education.
Ability to manage own work with limited supervision through the use of planning and time management showing initiative and positive attitude.
Demonstrate a practical understanding of Aboriginal and Torres Strait Islander inequities, and health issues to deliver informed, evidence based culturally appropriate care.
Remuneration value up to $117,787 p.a., comprising salary between $77,342 - $103,235 p.a., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (HWF3). Applications will remain current for 12 months. Please note: only applications from candidates will be accepted (applications that may result in an agency fee will not be considered).
