ONCALL | SACARE is currently looking for a dynamic Clinical Nurse Educator – who holds APHRA registration as an experienced Registered Nurse who is passionate about a client-centred approach to care. The Clinical Nurse Educator is involved in the provision of expert clinical assessment & care to participants, the effective delivery of high-level training services which encompasses a Registered Nurse Delegation of care model in disability services. The position is also responsible for the effective development and implementation of complex clinical care plans for participants with high needs in a disability setting.

About you:

AHPRA Registered Nurse or Enrolled Nurse with 5 years of Community Experience (unencumbered license)

Minimum Certificate IV in Training and Assessment or Graduate Diploma in Education (or willing to obtain)

Have a NDIS workers check, WWCC or willingness to obtain

Have a minimum of 5 years of nursing experience, with an in-depth understanding of community nursing

Be able to develop and manage mandatory education in consultation with the General Manager of Clinical and Therapeutic Services

Identify and respond to the learning needs of individuals and the delivery of service
-
Hands-on clinical leader, with demonstrated evidence of providing great client service
-
Can assess client needs and create care plans in collaboration with the client and stakeholders which are clear, concise and meets their needs.

Have a demonstrated knowledge of advanced nursing supports, including wound care, gastrostomy care, chronic conditions management

Have a sound understanding of the NDIS & other funding streams i.e., DSOA, LSA etc- essential

Experience in the disability or health sector and an understanding of the issues experienced by people with disabilities and their families – essential

Contribute to, and maintain where required, data collection for statistical purposes and collate where required for presentation to the relevant management stream.

Provide enhanced clinical service suggestions and implement education framework
