At Central Coast Local Health District (CCLHD), we are committed to Trusted Care, Better Health for Everyone, and we are looking for an experienced Social Worker Level 3 - Senior Clinician to join our new Palliative Care unit based at Wyong Hospital.
About the Opportunity
This position is based in the Palliative Care Unit at Wyong Hospital and provides advanced clinical social worker services within the inpatient setting, supporting patients with life-limiting illness and their families during acute stages of care. You will play a key role in supporting complex decision making, advance care planning, and end of life care within the inpatient setting.
In this role you will:
- Undertake complex psychosocial assessments and develop comprehensive care plans.
- Provide counselling, grief and bereavement support to patients and their families.
- Support advanced care planning and facilitate ethical decision-making.
- Collaborate effectively with the multidisciplinary team to deliver person-centred care.
- Provide clinical supervision, mentoring and support to students and staff.
- Contribute to service development, quality improvement initiatives and policy implementation.
- Liaise with community services and external stakeholders to promote seamless continuity of care.

About You
We are looking for someone who has:
- Tertiary qualifications in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
- Significant clinical social work experience, with advanced knowledge of psychosocial care in complex health settings, ideally within palliative care or acute inpatient services.
- Demonstrated experience providing person-centred, trauma-informed and culturally responsive assessment and intervention for patients and families experiencing complex health challenges.
- Excellent communication, leadership and stakeholder engagement skills, with the ability to supervise, mentor and work collaboratively within a multidisciplinary team.
- A commitment to evidence-based practice, continuous improvement and delivering compassionate, high-quality care.
