We are seeking an experienced and motivated NDIS Support Coordinator to join our growing team. This role is ideal for someone with a strong background in case management who is passionate about supporting people with disability to navigate the NDIS and achieve their goals.
As a Support Coordinator, you will work closely with NDIS participants, their families, and service providers to ensure participants receive the right supports, build capacity, and maintain choice and control.
Key Responsibilities
- Provide Support Coordination in line with NDIS guidelines
- Assist participants to understand and implement their NDIS plans
- Coordinate and link participants with appropriate services and supports
- Undertake comprehensive assessments, goal planning, and risk management
- Monitor plan utilisation and support participants through reviews
- Maintain accurate case notes, reports, and documentation
- Advocate for participants to ensure their needs and goals are met
- Build and maintain strong relationships with participants, families, and stakeholders
Essential Criteria
- Minimum 1–2 years’ experience in case management (disability, community services, mental health, or related field)
- Sound understanding of the NDIS framework and service system
- Strong organisational and time management skills
- Excellent written and verbal communication skills
- Ability to work autonomously and manage a varied caseload
- High level of professionalism, empathy, and problem-solving ability
- NDIS Worker Screening Check (or willingness to obtain)
- Working with Children Check (if applicable)
- Current Australian Driver’s Licence and access to a vehicle
Desirable Criteria
- Experience as an NDIS Support Coordinator or Specialist Support Coordinator
- Relevant qualification (Community Services, Social Work, Psychology, Disability, or similar)
- Experience working with complex needs and multiple stakeholders
