About the Role
As a Lifestyle Facilitator (Support Worker), you will play a vital role in supporting people living with disability to achieve their personal goals and participate fully in their communities.
Working within a person-centred and strengths-based framework, you will provide tailored supports that promote independence, choice, inclusion, and overall wellbeing.
In this role, you will:
- Support Participants to develop life skills across daily living, social, recreational, and employment-related activities
- Encourage choice, autonomy, and meaningful decision-making
- Facilitate community access and social inclusion
- Deliver supports that uphold dignity, respect, and individual rights
- Foster confidence, independence, and personal growth

- Availability for morning shifts and weekdays

- Ability to work independently and as part of a team
Skills and Experience
- Experience supporting people with disability and/or mental health conditions
- Confidence supporting individuals with complex support needs and behaviours of concern
- Strong communication and interpersonal skills
- Experience working active overnight shifts
- Competency in manual handling techniques
- Ability to mentor, guide, and positively influence others
Mandatory Requirements
Applicants must hold:
- Current Queensland Green P-Plate or Open Driver Licence
- Fully comprehensive vehicle insurance and access to a reliable vehicle
- Current Blue Card (Working with Children Check)
- Current NDIS Worker Screening Clearance (Yellow Card)
- Completion of the NDIS Worker Orientation Module
- Current First Aid and CPR Certification
- Evidence of COVID-19 vaccination in accordance with current organisational requirements
Desirable Qualifications and Experience
- Certificate III, Certificate IV, or higher qualification in Disability, Community Services, Mental Health, or a related field
- Experience working in Supported Independent Living (SIL) environments
- Knowledge of Positive Behaviour Support Plans (PBSPs)
- Understanding of privacy, confidentiality, and professional boundaries
- Experience building collaborative relationships with families, allied health professionals, and external service providers
Key Responsibilities
- Support Participants to achieve goals outlined in their NDIS Plans
- Implement Positive Behaviour Support strategies and skill-building activities
- Assist with personal care, including showering, dressing, grooming, and toileting
- Perform manual handling tasks, including hoist transfers and wheelchair support
- Facilitate community participation and social engagement
- Assist with household tasks, meal preparation, and maintaining a safe living environment
- Collaborate with families, support networks, allied health professionals, and stakeholders
- Maintain accurate records and documentation
- Adhere to all abbaCare policies, procedures, and relevant industry standards
