In this pivotal role, you’ll deliver consumer‑directed care that puts choice, control, and quality of life at the centre of everything you do—supporting individuals to truly live life their way.
About the Role
As a Care Partner, you will:
- Conduct comprehensive assessments and develop personalised care plans and budgets
- Provide information, referrals, and advocacy to connect clients with the right supports
- Collaborate with health professionals and community networks to deliver holistic care
- Maintain accurate documentation and uphold high standards of service delivery
- Support and guide Care Workers and internal teams to ensure coordinated, client‑focused care
- Take responsibility for all aspects of care coordination and budget management for your client caseload
What You’ll Bring
To succeed in this role, you’ll bring:
- Qualification in aged care or a related field
- Experience in aged care or disability services, ideally within the Home Care Package program
- Strong budgeting and financial management capability
- Excellent communication and interpersonal skills
- A current First Aid Certificate, valid driver’s licence, and access to a comprehensively insured vehicle
- Confidence using client management systems and a strong commitment to WHS practices
- Fast learner, willing to be mentored in the role
