Headstart Acquired Brain Injury Services is a specialist service supporting the choices and independence of people with an acquired brain injury (ABI). We are dedicated to supporting individuals with an ABI in enhancing their life skills and community engagement. Our core purpose is to make a difference in the lives of people touched by brain injury by providing opportunities and inspiring hope. We are seeking a compassionate Care Coordinator for a part-time, 2-year fixed-term contract, working Monday, Tuesday, Thursday, and Friday.
Playing an integral role in supporting people with an ABI to pursue activities of choice in the community and re-develop independent life skills, you are a motivated and resourceful person who works creatively supporting people with an ABI to become more independent in life and achieve their goals.
In this role, you will manage a caseload, ensuring shifts and rosters align with each consumer’s budget, goals and plan. You will work closely with Community Support Workers (CSWs) to deliver high-quality care and maintain ongoing communication with the consumer's Support Coordinator and other relevant stakeholders.
Your commitment to the concept of individualised supports, community inclusion and consumer driven services will be evident. This role will involve supervision and rostering of Community Support Workers, and you will participate in an on-call system as required.
Please note: This is not a Support Coordinator position as defined under the NDIS. In this role, you will work directly with participants to ensure they receive high-quality attendant care services. Your focus will be on building strong relationships with the consumer and their Support Coordinator, while our team provides the direct care and support needed for each individual.

Desired skills and experience:
- Teritary qualification in Health, Welfare or similar
- Exhibit robust case management skills, adept at prioritising and managing intricate caseloads efficiently.
- You would ideally have experience in the ABI sector and community development skills, or as at least understand the effects of brain injury on the psychological, social, physical and cognitive functions.
- Exceptional communication skills, both verbal and written, are imperative.
- Your well developed and demonstratable interpersonal skills including leadership negotiation and conflict resolution will be well utilised in this role
- Proficiency in the NDIS Quality and Safety Framework
- Proven ability to operate effectively both independently and collaboratively within a team environment.
- Strong organisational skills, including the capability to plan, prioritise workloads, and multitask
- Current Provide First Aid certificate HLTAID011 (or willingness to obtain).
- Completion certificate for the NDIS Worker Orientation Module
- Proficiency in computer usage, particularly in Microsoft Office applications (Word, Outlook, Excel)
- Cleared Working with Children Check and NDIS Worker Screening Check (or willingness to obtain).
- Possession of a current NSW Driver’s License
- Documentation confirming your Right to Work in Australia.
