About the role
As a Registered Nurse – Care Partner, you’ll use your clinical expertise in a different way — bringing together your assessment skills, critical thinking, and clinical judgement to coordinate and oversee care for your own portfolio of residents. This is a nurse navigation role where you will build meaningful relationships, take a holistic view of each resident’s needs, and guide their care journey. In this role, you will:
- Get to know your residents and understand what matters most to them
- Complete holistic assessments and develop meaningful, person-centred care plans
- Work closely with GPs, allied health professionals, and care teams to coordinate support
- Help residents and families to understand and navigate their care options
- Monitor changes and adapt care approaches to support wellbeing over time
Role responsibilities
- Coordinate resident assessments, care planning, reviews and evaluations
- Build trusted relationships with residents and families, monitoring wellbeing
- Explain care and funding options clearly and compassionately
- Connect residents with health and support services as required
- Collaborate with GPs, Nurse Practitioners, allied health and external providers
- Ensure accurate documentation and use of relevant systems
- Conduct regular resident and family check-ins
- Complete Move In assessments and participate in case conferences
- Respond to emergencies and manage clinical risks
- Contribute to quality improvement, onboarding and positive team culture
Qualifications, Experience and Skills
- Registered Nurse with minimum five (5) years clinical and/or community care experience
- Current Registration with AHPRA.
- Are passionate about providing high-quality, person-centred care
- Feel confident using their clinical judgement and critical thinking
- Value taking time to build relationships with residents and families
- Are interested in stepping into a coordination and nurse navigation role
- Have experience in, or a strong interest in developing, dementia care practice
- Want to make a real difference to how care is experienced
- Demonstrate a commitment to continuous professional development
- Knowledge of Support at Home program and aged care funding systems
