Where you'll be working
The successful candidate will be joining an enthusiastic multi-disciplinary team consisting of Child and Family Health Nurses, Allied Health Clinicians and Administration Support all working toward supporting vulnerable families from pregnancy to child’s age of 2yrs. The position is located at Taree (Lower Mid North Coast) however, you will be a part of the SNF Port Stephens / Lower Mid North Coast (LMNC) team. This will mean travelling to Raymond Terrace and Bulahdelah at times to meet with the team. The role requires home visits to families and weekly participation in case conferencing.
The Opportunity
As the Social Worker within the Sustaining NSW Families (SNF) multidisciplinary team, you will play a pivotal role in supporting families from pregnancy through to their child's second birthday. Working in close partnership with Child and Family Health Nurses and other allied health professionals, you will provide specialist social work expertise to help families achieve the best possible outcomes during this important stage of life. You will be a valued member of a collaborative team, participating in weekly multidisciplinary case conferences and being introduced to every family enrolled in the program. This provides a unique opportunity to build meaningful relationships and deliver early intervention and family-centred care that makes a lasting difference. Reporting to the SNF Port Stephens/LMNC Nurse Unit Manager (NUM), you will be well supported by the Manager of Child and Family Health Services, the SNF Port Stephens/LMNC NUM/Coordinator, the SNF Allied Health team, and Child and Family Health Nurses. A comprehensive orientation will be provided to help you settle into the role, along with ongoing professional support and opportunities to maintain mandatory education requirements.
About you
The successful candidate will be enthusiastic and have significant demonstrable experience working with children and families to enable them to work autonomously, utilise independent clinical reasoning, attend psychosocial assessments and advocate on behalf of the families.
Requirements

Holds as a minimum a bachelor’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.

Eligibility to drive in NSW and willingness and ability to travel for work purposes.

Demonstrated extensive clinical experience providing social work services to vulnerable families with complex clinical presentations such as perinatal mental health, child protection, domestic violence and substance misuse.

Demonstrated capacity to provide clinical consultation in partnership with the child and family health nurse and facilitate case review, in the context of family centred casework within a community based multidisciplinary team

Demonstrated ability to provide information, advice and education to clients, carers and other professionals including: anticipatory guidance, treatment intervention, and management strategies to facilitate progress and promote optimal development of the child.
