About the Role
Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
The Assertive Community Outreach Team (ACOT) is a specialist outreach service that provides short-term, intensive support (up to 12 weeks) to adults with alcohol and other drug use issues who are experiencing barriers to accessing treatment and have limited support networks. The primary aim of the service is to stabilise clients and establish sustainable treatment and support pathways.
The Social Worker Level 3 will deliver outreach-based clinical and psychosocial interventions across a variety of settings including clients' homes, hospitals, community health centres, mental health services, non-government organisations, and other community locations. The role involves comprehensive psychosocial assessment, care coordination, treatment planning, advocacy, referral and linkage to services, addressing barriers to service access, and supporting clients with issues such as homelessness, mental health concerns, domestic and family violence, child protection, legal matters, Centrelink, NDIS, and complex health and social needs. Practice is guided by harm minimisation, trauma-informed care, and person-centred principles.
The position works within a multidisciplinary team framework alongside Social Workers and a Clinical Nurse Consultant. Clinicians contribute collectively to client care, participate and collaborate extensively with internal and external stakeholders across Sydney Local Health District and the community sector. Clinical work is delivered on an outreach basis across the SLHD catchment and therefore requires regular travel throughout the district.

Ideal Candidate
- Must hold a Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers, with Current NSW Driver’s Licence (P1 and P2 acceptable).
- Advanced clinical knowledge, skill and significant clinical experience in the provision of social work, with experience at Level 2 or higher.
- Experience in the provision of clinical supervision, mentoring and development of junior staff and students.
- Experience working with people experiencing alcohol and other drug use issues, mental health concerns, homelessness, trauma, and other complex psychosocial needs is highly desirable.
- Experience in outreach, case management and interagency collaboration will be well regarded.
