Divine Support Care is seeking a Service Delivery Manager to oversee participant supports across Brisbane. This role combines service delivery, rostering oversight, and frontline team leadership, ensuring high-quality, compliant service delivery.

About the Role
- Coordinate and implement NDIS-funded supports aligned with participant goals
- Oversee and manage staff rosters to ensure continuity of care and coverage
- Provide day-to-day leadership and guidance to support workers
- Liaise with participants, families, allied health, and external providers
- Manage referrals, service agreements, reviews, and plan utilisation
- Monitor service delivery, identify risks, and escalate concerns appropriately
- Maintain accurate case notes, rostering records, and compliance documentation
Essential Requirements:
- Cert IV or Diploma in Disability, Community Services, Mental Health (or similar)
- Experience leading support workers and managing rosters
- Strong understanding of NDIS legislation and Practice Standards
- NDIS Worker Screening Check
- First Aid & CPR
- Driver’s Licence and reliable vehicle
- Australian work rights
Pay
- Paid in line with the SCHADS Award (QLD)
- Indicative classification: SCHADS Level 3–4, depending on experience
