Working days: Monday, Tuesday and alternate Wednesdays.
About Role:
The successful applicant will have demonstrated advanced clinical skills in acute/subacute stroke and neurological rehabilitation to provide evidence-based assessments and interventions aligned with best practice guidelines. They will provide leadership within this clinical area and support the P3 Clinical Lead Occupational Therapist in overseeing the Occupational Therapy stroke team and service. This role requires a strong understanding of stroke rehabilitation model of care together with excellent organisational skills.
About You
To be successful in this role you must provide statement addressing the following selection criteria:

Demonstrated advanced clinical knowledge and ability to apply clinical reasoning, sound judgement and evidence-based practice in the assessment, management and rehabilitation of patients with stroke and neurological conditions.

Demonstrated ability to coordinate the care of patients with complex stroke and neurological conditions, including discharge planning, risk management and navigation of health and community services.

Demonstrated ability to communicate and collaborate effectively with multidisciplinary team members, patients, families, carers and external stakeholders to support high-quality, patient-centred care.

Demonstrated leadership, supervision and mentoring skills, with well-developed communication and interpersonal skills that support effective teamwork, education and clinical service delivery.
Desirable Criteria

Knowledge of, and experience with, the NDIS and associated referral pathways.

Experience participating in or leading quality improvement, service evaluation and/or research activities.
