About the Opportunity
We are seeking motivated Registered Nurses to join our dedicated and high-performing Ambulatory Care team at Griffith Base Hospital. Our service delivers innovative models of care through Hospital in the Home (HITH), an outpatient clinic (PIXI- procedures, investigations and infusions) and a newly established service, Aged Care Outreach Service (ACOS), providing hospital-level care in the hospital clinic or the patients' home. This rewarding role offers the opportunity to deliver person-centred care that helps to reduced demand for hospital services, ED avoidance and to promote multidisciplinary care to support patients to remain at home and connected to their community.
As the Registered Nurse, you will:
Provide person-centred, evidence-based nursing care in accordance with professional standards and legislation.
Conduct patient assessments, contribute to care planning, and evaluate outcomes to support safe and effective care delivery, across HITH, PIXI and ACOS programs.
Collaborate effectively with multidisciplinary teams to drive optimal patient outcomes and ensure seamless care delivery.
Participate in continuous quality improvement activities and promote a culture of safety and learning.

About You
Our ideal candidate will demonstrate:
- Solid, acute care nursing skills and knowledge with at least 4 years of nursing experience
- Experience in community, aged care or hospital in the home services (desirable but not essential)
- Strong clinical reasoning skills with the ability to prioritise and case management skills.
- Proven ability to work independently and across multidisciplinary teams, with a commitment to mentoring and supporting team members.
- Exceptional communication and interpersonal skills.
- A strong commitment to person-centred care, cultural safety, and ongoing professional development.
