About the role
As the Registered Nurse in our Geriatric Evaluation and Management/REHAB@HOME service, you will provide high-quality nursing care to patients in their own homes, supporting the delivery of care within a community setting.
You will play a key role in maintaining continuity of care, working as an autonomous practitioner while collaborating closely with a multidisciplinary team. This role requires you to coordinate and deliver advanced clinical care in accordance with Eastern Health's policies, procedures, standards, and professional guidelines. Care provided may include medication management, wound care, clinical assessments, patient observations, bladder scans, ECGs, pathology collection, and other nursing interventions as required.
You will be part of a large multidisciplinary team comprising Geriatricians, Specialist Rehabilitation Physicians, Occupational Therapists, Physiotherapists, Allied Health Assistants, Speech Pathologists, and Dietitians, working together to achieve the best possible outcomes for patients.
About you
To be successful in this role you will possess:
Current Division 1 Nursing registration with AHPRA
A minimum of 5 years' post-registration nursing experience in an acute medical/surgical setting or a relevant specialty area such as ICU, ED, CCU, Sub-Acute, Medical, or Community Nursing.
Demonstrated ability to work autonomously, showing initiative and self-direction while contributing effectively within a multidisciplinary team.
Well-developed clinical assessment, documentation, and critical thinking skills, including comprehensive patient assessment, pathology result interpretation, and clinical decision-making.
Experience in a range of nursing interventions, including wound management (acute and chronic), male and female urinary catheterisation, medication management, and clinical monitoring.
A commitment to maintaining and advancing professional knowledge and skills relevant to the role.
A current Victorian Driver Licence.
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
