The Gold Coast Case Manager & Community Growth Lead is responsible for establishing EHCS’s presence on the Gold Coast by sourcing new clients, building referral pathways, and delivering high‑quality client assessments and care plans. This role is ideal for an RN or experienced health professional seeking flexible work that will expand as the client base grows.
Key Responsibilities
- Client sourcing — Identify, engage, and onboard new clients across the Gold Coast.
- Referral network development — Build strong relationships with GP clinics, allied health providers, building managers, retirement villages, and community groups.
- Local client assessments — Conduct in‑home assessments, develop care plans, and ensure documentation meets EHCS standards.
- Client relationship management — Act as the local point of contact, ensuring clients feel supported and connected.
- Community presence — Attend local seniors’ events, wellness groups, and community activities to raise awareness.
- Quality & compliance — Maintain accurate notes, incident reporting, and care documentation aligned with EHCS policies.
- Collaboration with Sydney HQ — Work closely with Sydney operations for rostering, HR, invoicing, and incident management.
Skills & Experience
- Registered Nurse or experienced health professional (EN, AHA, Social Worker, Case Manager).
- Strong communication and relationship‑building skills.
- Ability to work independently and represent EHCS professionally.
- Experience in aged care, community care, or disability support.
- Confidence engaging with GPs, allied health, and community organisations.
- Understanding of care planning, assessments, and client documentation.
