Position Available: Community Care Nurse / PCA – Diabetes Support (Thomastown)
We are seeking a qualified and experienced Community Care Worker (RN/PCA) to commence immediate services for a new client in Thomastown.
Commencement: From tomorrow
Location: Thomastown
Schedule: Daily (Monday to Sunday)
Time: Between 12:00 PM – 1:00 PM
Pay: $730 total
Duties Include:
- Reviewing Libre 2 Sensor glucose readings
- Blood Glucose Level (BGL) monitoring and documentation
- Administration of prescribed insulin in accordance with the Diabetes Management Plan
- Monitoring client condition and escalating concerns as per care plan
- Completion of accurate progress notes after each visit
Requirements:
- Experience in diabetes care and insulin administration
- Strong understanding of BGL monitoring and documentation
- Ability to follow clinical care plans accurately
- Current clinical qualifications (RN or suitably qualified PCA with medication competency)
- Strong communication and escalation skills
- High attention to detail and patient safety focus
Important Notes:
- Must review all referral documents, discharge summary, health summary, and diabetes management plan prior to first visit
- Immediate start required
- Must follow all clinical and WHS procedures strictly
