EmpoWRreach by WoundRescue is our dedicated community nursing service, delivering high-quality, person-centred care to people in their homes across New South Wales. As part of the WoundRescue family, our clinicians provide holistic nursing care that supports individuals to remain safe, independent and well within their own communities.
Our services span chronic disease management, post-acute care, continence, medication support, disability and aged care, health assessments, and advanced wound and skin management. We work collaboratively with general practitioners, specialists, hospitals, aged care providers, NDIS participants and families to deliver coordinated, evidence-based care.
We are currently seeking compassionate and motivated RNs, ENs and CertIV to join our growing community nursing team across the Hunter and Central Coast region. This role involves providing high-quality nursing care in clients' homes, promoting independence, improving health outcomes and delivering an exceptional patient experience.
Experience in community nursing is highly desirable. Experience working with people living with spinal cord injury (SCI), disability, complex clinical needs or advanced wound management will be highly regarded, however support and ongoing professional development will be provided for the right candidate.
About the Role:
As a member of the EmpoWRreach by WoundRescue team, you will deliver high-quality, person-centred care to clients in their homes across the Hunter and Central Coast region. Working within a supportive multidisciplinary team, you will manage your own community caseload, build strong relationships with clients and healthcare providers, and contribute to improving health outcomes through compassionate, evidence-based care.

Key Responsibilities:
✅ Undertake care assessments, implement treatment plans and monitor progress
✅ Work closely with our Wound specialist to escalate concerns and adjust care as needed
✅ Document clinical care and maintain accurate and timely records
✅ Assist with referrals and service coordination where required
Skills & Experience:
✔ Current AHPRA registration (Registered Nurses and Enrolled Nurses only)
✔ Previous experience in community nursing is essential
✔ Experience in wound assessment and management
✔ Experience providing catheter care, continence management and medication administration (including cytotoxic medications where within scope of practice)
✔ Excellent clinical assessment, communication and documentation skills
✔ Ability to work autonomously while contributing to a multidisciplinary team
✔ Current unrestricted driver's licence and reliable motor vehicle (travel is required)
