Where you'll be working
Chronic and Complex Allied Health services support older people to maintain independence and remain living safely in the community.
Community-based allied health service across WSLHD
Multidisciplinary Chronic and Complex team
Reports to Service Coordinator and Program Lead
Provides services in clients’ homes and via telehealth
Collaborates with carers and external service providers
What you'll be doing
You will provide occupational therapy services to clients with complex health and functional needs.
Conduct comprehensive assessments including functional and safety risks
Develop and implement client-centred care plans
Manage a clinical caseload and prioritise service delivery
Liaise with multidisciplinary teams and external providers
Participate in quality improvement and evidence-based practice

Essential Criteria:
- Qualification: Tertiary qualifications in Occupational Therapy
- Registration: Current general Occupational Therapist registration with AHPRA
Selection Criteria:
1. Demonstrated skills and experience in Occupational Therapy assessment, planning and evaluating interventions for clients with chronic and complex health conditions.
2. Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community.
3. Demonstrated ability in the use of relevant evidence based best practice guidelines to guide decision making for best client outcomes.
4. High-level written and oral communication skills, computer competency and ability to use electronic medical records, word processing and databases.
5. Ability to work independently and as part of a multidisciplinary team.
6. Current Drivers Licence valid within Australia.
