Your new role:
As the Clinical Nurse Consultant - Diabetes Unit, you will provide advanced clinical expertise, leadership and consultancy in diabetes education and management across community and home-based healthcare settings. Working collaboratively within a multidisciplinary team, you will play a key role in delivering safe, evidence-based and person-centred care to patients with complex chronic disease needs.
Key Responsibilities
Deliver advanced clinical diabetes nursing care, assessment, education and care coordination across the continuum of care.
Provide clinical leadership, mentorship and specialist consultancy to nursing and multidisciplinary teams.
Lead quality improvement, evidence-based practice and service development initiatives to enhance patient outcomes.
Support hospital avoidance strategies through community-based chronic disease management and patient education.

You are a compassionate and experienced nursing professional with advanced diabetes nursing knowledge and a strong commitment to person-centred care.
