What you'll be doing
The Clinical Leader provides Clinical Governance and oversight for Southern NSW Local Health District (SNSW LHD) My Aged Care Assessment Services (MACAS) including:
- SNSW LHD Aged Care Assessment Program (ACAP) assessors.
- Regional Assessment Service (RAS) Assessors.
- Support for new staff/students
To provide a people focused quality service in collaboration with the multidisciplinary teams.
Selection Criteria
- Tertiary qualifications in Allied Health or Nursing and Registration with the Australian Health Practitioner Regulation Agency (Nursing, Occupational Therapist, Physiotherapist and Psychologist) or eligibility for membership with the peak professional association (Social Workers, Speech Pathologist or Dietician). For Nursing at least 5 years post registration experience and Post-graduate qualifications and at least 3 years’ experience working with frail older consumers and those with chronic illness in an ACAP role or current enrolment in or commitment to enrol in an approved postgraduate course within the next 12 months. For Allied Health Extensive specialist or broad generalist knowledge working with frail older consumers and those with chronic illness in an ACAP role. Statement of Attainment 4 (SOA4) training in My Aged Care (MAC) or equivalent.
- Knowledge of the MAC and Australian Government Guidelines for the Regional Assessment Service (RAS) and Aged Care Assessment Program (ACAP) specifically relating to the eligibility requirements for the streamlined assessment services, consumer consent and approval for entry to an Aged Care Place together with a knowledge of the eligibility criteria and referral process for a range of post-discharge options of care for older people.
- Demonstrated ability to provide supervision and support to My Aged Care Assessment Services (MACAS) Assessors, and experience in assessment, ACAP delegation and management of older people in a community-based MACAS.
- Demonstrated ability to think critically, problem solve and make timely and sound clinical decisions whilst working autonomously and when supervising others within a multidisciplinary team.
- Ability to demonstrate excellent interpersonal and communication skills, including written, verbal, computer skills and proficiency in the use of the MAC and CHOC applications.
- Demonstrated ability to communicate with, consult and negotiate effectively with the community, health professionals, consumers and their families as well as to develop and present education programs to these groups.
- Demonstrated knowledge and understanding of management principles, research, strategic planning, and policy development.
Evidence of a current unrestricted drivers’ licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances and overnight stays.
