We are seeking an experienced Clinical Care Coordinator to join the team at Greenwood Manor in Dingley Village on a fixed-term contract, from 30/09/2026 - 06/11/2026
As Clinical Care Coordinator, you will play a key role in supporting the delivery of high-quality clinical care and fostering a culture of excellence across the home. Working closely with the Home Manager, you will lead, support and mentor the clinical and care teams to ensure every resident receives safe, personalised and dignified care.
This is an excellent opportunity for an experienced Registered Nurse who is passionate about aged care, clinical leadership and continuous improvement, and who enjoys supporting others to achieve positive outcomes for residents.

Lead and support clinical care planning, documentation and delivery to meet each resident's individual needs

Champion person-centred care and ensure residents and families are involved in meaningful care decisions

Provide guidance and support to clinical staff regarding AN-ACC assessments and accurate, timely documentation

Support and mentor Registered and Enrolled Nurses in their daily clinical responsibilities

Participate in case reviews and monitor resident care outcomes

Conduct regular clinical audits and ensure identified gaps are promptly addressed

Support the Home Manager with clinical governance, compliance and continuous improvement initiatives

Promote a strong culture of resident safety, quality care and accountability

Provide coaching, education and clinical guidance to staff to build confidence and capability

You will be an experienced Registered Nurse with strong clinical knowledge and a genuine passion for aged care and developing others.
You will ideally have:

Current AHPRA registration as a Registered Nurse (Division 1)

Proven clinical leadership experience within residential aged care or a senior clinical position

Strong understanding of aged care best practice, AN-ACC and clinical documentation requirements

Excellent communication, leadership and interpersonal skills

Strong organisational and time-management skills

Confidence using technology, Microsoft Office and clinical care systems

A commitment to continuous improvement, quality and resident outcomes

The ability to lead by example and build positive relationships across the home
