We are looking for Community Worker who can work within a dynamic and high performing environment.
· Assess client needs and provide tailored support in accessing essential services such as health care, housing, employment, and social services.
· Design, coordinate, and assist in implementing community engagement and development initiatives that address social inequalities and promote well-being.
· Work collaboratively with families, health professionals, government agencies, and local stakeholders to deliver integrated and client-centred services.
· Maintain accurate and up-to-date records, reports, and client case notes in accordance with organisational policies, regulatory guidelines, and funding requirements.
· Facilitate referrals to relevant service providers and ensure consistent follow-up to support continuity of care.
· Monitor and evaluate the effectiveness of community programs and interventions, making recommendations for improvements as necessary.
· Promote social inclusion, equity, and respect for cultural and individual diversity in all aspects of service delivery.
· Advocate on behalf of clients to ensure their voices are heard and their rights are upheld within the community and relevant service systems.
· Support clients in developing life skills, increasing independence, and building capacity to engage with their community
Job Requirements
Minimum 1 year relevant experience, sound NDIS knowledge, service-focused, strong communication skills, able to work independently and within a team.
An understanding of NDIS and a commitment to its principles.
Service-oriented and able to work both autonomously and collaboratively within a team
Excellent interpersonal and communication skills
Ability to work both independently and as part of a multidisciplinary team.
Job Types: Full-time, Permanent
Pay: $77,000.00 – $85,000.00 per year
