Are you an experienced Renal Nurse looking to make a significant impact within regional communities?
About the role:
A Chronic Kidney Disease (CKD) nurse in the Murrumbidgee Local Health District supports patients to live well with kidney disease. You will help to monitor kidney function, slow disease progression, and coordinate care alongside nephrologists and GPs. You will work within our CKD team based at Deniliquin or Griffith and will provide telehealth and home visits throughout western MLHD.
Patient Education & Support: Educating patients and their families about the stages of CKD, symptom management, and making informed choices regarding treatment options (including dialysis or conservative supportive care).
Care Coordination: Working closely with the multidisciplinary team (Renal Nurse Practitioners, Dietitians, Social Workers, and Aboriginal Health Workers) to ensure seamless care across both hospital and community settings.
Clinical Monitoring: Assessing fluid status, evaluating blood pressure, interpreting pathology (eGFR, creatinine, electrolytes), and monitoring disease progression.
Self-Management: Helping patients build practical skills to manage their diet, medications, and lifestyle.
