Your new role:
As a Physiotherapist with Rehab@Home, Metro South Health@Home, you will provide high-quality, evidence-based physiotherapy services to patients in community settings, supporting their rehabilitation, independence and transition from hospital to home.
Deliver comprehensive physiotherapy assessment, intervention, education and evaluation, including care coordination and support for discharge planning within an interprofessional team.
Manage an allocated caseload, applying sound clinical reasoning, prioritisation and increasingly independent clinical judgement while maintaining accurate clinical documentation and service reporting.
Build strong partnerships with patients, carers, families and multidisciplinary colleagues, contributing to clinical case conferences, team meetings and consumer-centred care planning.
About you:
We are seeking a motivated and compassionate physiotherapist who can combine sound clinical practice with strong communication and collaboration skills to deliver safe, patient-centred rehabilitation in the community.
You are a registered Physiotherapist with the Physiotherapy Board of Australia (AHPRA) and demonstrate contemporary clinical knowledge and an evidence-based approach to physiotherapy practice.
You have strong clinical reasoning, communication, interpersonal and organisational skills, with the ability to manage a caseload, prioritise competing responsibilities and work effectively within an interprofessional team.
You are committed to patient-centred and culturally responsive care, engaging patients, carers and families in shared goals and supporting their physical, psychological, social and emotional wellbeing.
