Rose Aged Care Service is a registered, customer-focused care provider based in Blacktown, delivering personalised, compassionate, and ethical support services across the Sydney metropolitan area. As our business continues to grow, we are seeking an experienced and dedicated Support Coordinator to join our team on a part-time basis.
Position Purpose:
As a Support Coordinator, you will coordinate and manage the delivery of high-quality, person-centred NDIS services while supporting clients and their families. You will be responsible for developing and reviewing support plans, coordinating service delivery, ensuring compliance, and building strong relationships with clients, families, service providers, and staff. Your role is essential in ensuring clients feel supported, safe, empowered, and respected throughout their time with Rose Aged Care.
Key Responsibilities:
- Conduct comprehensive client assessments and develop tailored, person-centred NDIS support plans.
- Guide clients and families in using NDIS funding effectively to support wellbeing, independence, and quality of life.
- Provide support coordination services, assisting clients to access appropriate services, navigate the NDIS system, and exercise choice and control over their supports.
- Build and maintain strong, positive relationships with clients, families, staff, external service providers, and community networks.
- Ensure consistent, high-quality service delivery through collaboration with clinical teams, allied health professionals, and support staff.
- Support clients in accessing additional community services, supports, and advocacy as needed.
- Monitor and regularly review support plans, making adjustments as client needs and goals evolve.
- Maintain accurate, timely documentation and ensure compliance with NDIS Practice Standards and organisational policies.
- Supervise, mentor, and support support workers to promote safe, ethical, and effective practices.
- Actively participate in advertising, networking, and community outreach activities to promote Rose Aged Care Service and assist in attracting new NDIS clients.
- Contribute to a respectful, inclusive, and positive team-focused work environment.
Essential Criteria:
- Relevant qualifications in Community Services, Disability, Health, or Social Work (Diploma level or above preferred).
- Minimum 3 year of experience in a similar role involving support coordination.
- Strong knowledge of NDIS and community-based support models.
- Experience in or willingness to engage in client outreach, marketing, and referral-building activities.
- Empathetic, solutions-focused, and able to build rapport quickly with clients from diverse backgrounds.
- Excellent communication, time management, interpersonal, and documentation skills.
- Demonstrated ability to lead, supervise, and support a care or support team.
- Proficient in using digital systems, client management software, and support coordination tools.
- Current driver's licence and ability to travel to client homes across the Sydney metropolitan area.
- Current Police Check, First Aid & CPR certification, and NDIS Worker Screening Check (or willingness to obtain).
Pay: Up to $35.00 per hour
