We’re looking for a Care Coordinator who is organised, proactive, people-focused, and passionate about helping participants achieve their goals. This is a diverse role that combines participant engagement, service coordination, administration, problem-solving, and stakeholder liaison.
You’ll be the central point of contact for participants and families, helping to ensure supports are delivered effectively, documentation is maintained, problems are resolved, and participants receive high-quality services that align with their goals and aspirations.

About the Role
As a Care Coordinator, you will support participants throughout their journey with MarJen Collective. You’ll work closely with participants, families, support workers, allied health professionals, schools, support coordinators, Child Safety, and other stakeholders to ensure supports are delivered successfully.
This is a varied role where no two days are the same. One day you may be onboarding a new participant, the next you may be coordinating supports, preparing documentation, resolving service delivery issues, or working with families and stakeholders to achieve positive outcomes.
What You’ll Be Doing
● Supporting new participants through onboarding and commencement of services
● Building positive relationships with participants, families, and stakeholders
● Coordinating supports and service delivery to meet participant needs
● Preparing and maintaining participant documentation, service agreements, risk assessments, and support plans
● Working closely with support workers to ensure quality support delivery
● Managing participant records and maintaining accurate documentation
● Responding to service delivery issues and identifying practical solutions
● Reviewing participant feedback, incidents, and outcomes
● Monitoring participant goals and supporting positive progress
● Liaising with schools, Child Safety, allied health professionals, support coordinators, and other stakeholders as required
● Managing participant enquiries, emails, phone calls, and requests
● Supporting compliance, quality, and organisational processes
● Contributing to continuous improvement and positive participant outcomes

Essential Requirements
● Current Driver Licence
● Current NDIS Worker Screening Clearance, or ability to obtain
● Current Blue Card, or ability to obtain
● Current First Aid and CPR, or willingness to obtain
● Right to work in Australia
