As a Clinical Nurse in the Hospital to Home program, you'll play a vital role in ensuring patients transition safely from hospital into the community. From shaping personalised care pathways to working closely with multidisciplinary teams, you'll help reduce barriers, improve continuity of care and create better patient experiences across the region.
Every day, you'll contribute by:
Leading complex case coordination that enables safe, timely discharge and continuity of care
Partnering with multidisciplinary teams and community providers to deliver tailored patient outcomes
Applying clinical leadership and critical thinking to guide care planning and decision-making
Driving quality and safety improvements that enhance patient flow and service delivery

You're a confident and compassionate clinician who thrives in complex, fast-paced environments. You bring both clinical expertise and a strong sense of purpose, knowing that the work you do directly improves how people experience care beyond the hospital. Here's what will help you thrive in this role:
Current registration as a Registered Nurse with relevant professional accreditation
Strong experience in complex care coordination, discharge planning or case management
Advanced communication, problem-solving and stakeholder engagement skills
Ability to lead, influence and collaborate within multidisciplinary teams
You'll stand out from the crowd if you have postgraduate study relevant to clinical practice and experience working across hospital and community care settings.
Above all, you'll bring compassion, integrity, dignity, innovation and courage, helping deliver care that truly makes a difference.
