About the Role
We are seeking an experienced and motivated Team Leader – Service Coordination to lead a high-performing team within our Community Transitional Programs.
Reporting to the Head of Community Transitional Programs, you will lead a team of Service Coordinators to deliver exceptional client outcomes, ensuring services are responsive, high-quality, and aligned with organisational and funding requirements.
You’ll play a critical leadership role—coaching and developing your team, driving performance and accountability, and using data and insights to continuously improve service delivery and client experience.
Key Responsibilities
In this role, you will have responsibility for:
Leadership & Team Development
- Lead, coach and develop a team of Service Coordinators to achieve performance targets and KPIs
- Foster a collaborative, engaged and high-performing team culture aligned with Care Connect values
- Build capability across the team through mentoring, feedback and development planning
Service Delivery & Customer Excellence
- Ensure delivery of responsive, person-centred and high-quality services
- Support team members to manage client needs, feedback and complex situations
- Collaborate with internal teams to enhance the overall client experience
Operations & Performance
- Oversee day-to-day service coordination activities, including call and workflow management
- Monitor performance metrics, reporting and compliance with program requirements
- Ensure accurate data capture to support quality reporting and funding compliance
- Support workforce planning and operational efficiency
Continuous Improvement & Stakeholder Engagement
- Use data and insights to identify trends and drive service improvements
- Build and maintain relationships with key referral partners and stakeholders
- Contribute to program planning, innovation and service development
What You’ll Bring
To be successful in this role, you will demonstrate:
- Proven experience (3+ years) in a leadership role within a service delivery, call centre or human services environment
- Strong people leadership skills with the ability to coach, motivate and build high-performing teams
- Excellent communication, stakeholder engagement and influencing skills
- A strong focus on customer service and person-centred care
- Well-developed analytical and problem-solving skills with the ability to use data to drive outcomes
- Exceptional organisational skills and the ability to manage competing priorities
- Experience working in a regulated environment (aged care, health or community services highly desirable)
- Relevant tertiary qualifications in Health, Social Work, Community Services, Business or similar (desirable)
You will also require:
- Current driver’s licence and access to a reliable vehicle
- National Police Check and Working With Children Check
- Full working rights in Australia
