Make a real difference in the lives of older adults and vulnerable community members. We're seeking a compassionate and motivated Social Support Program Leader to join our team and help deliver engaging programs that promote independence, social connection and wellbeing.
Reporting to the Team Leader, you will assist in planning, developing and Facilitating centre-based and community programs align with a Wellness and Reablement approach. In this role, you will support participants to maintain their independence and wellbeing by delivering engaging programs and providing individual assistance where required. You will facilitate daily activities, monitor participant wellbeing, coordinate volunteers and transport services, maintain accurate records, and contribute to the continuous improvement of programs and services.
What we are looking for;
- Strong interpersonal, communication and organisational skills
- Ability to support frail aged and vulnerable clients with sensitivity to cultural diversity
- Experience planning and facilitating client-focused activities
- Ability to supervise volunteers and work within a multidisciplinary team
- Good IT skills, including client reporting systems and Microsoft applications
- Adhere to organisational values, policies and legislative requirements
To be successful, you will require;
- Victorian Drivers Licence
- A current Working with Children Check
- Holds or is working towards a Certificate IV or Diploma in Individual Support, Ageing Support and/or Leisure and Health (or willingness to obtain)
- Current First Aid Certificate Level 2
- Current CPR
- Ability to drive a 12-seater social support bus
