Female Disability Support Worker – After School Shifts

Shift Time: 2:45 pm – 4:45 pm, Monday to Friday
About the Role
Harmony House Community Care & Services is seeking a compassionate and reliable Female Disability Support Worker to provide after-school support for a young participant.
This role involves collecting the participant from school, providing support in the community and at home, and following strategies developed by the participant's care team and therapists to promote independence and positive outcomes.
Key Responsibilities
- Collect the participant from school.
- Provide safe transport to and from activities and home.
- Support community access and participation.
- Follow behaviour support plans and therapeutic strategies.
- Encourage independence, social interaction, and daily living skills.
- Build a positive, respectful, and professional relationship with the participant.
- Complete accurate shift notes and required documentation.
About You
To be successful in this role, you will have:
- Experience supporting people with disability, particularly children or adolescents (preferred).
- A caring, patient, and professional attitude.
- Strong communication and problem-solving skills.
- The ability to follow support plans and behaviour support strategies.
- A current Victorian Driver Licence and access to a reliable, insured vehicle.
- A valid NDIS Worker Screening Check.
- A valid Working with Children Check
- Current First Aid and CPR certificates.
