About the Role
Lifest HealthCare is seeking an experienced Support Worker to provide ongoing support to participants across Alexandra Hills and Woolloongabba.
The participants supported through this role have a combination of mental health and physical support needs, so we are looking for someone who is confident, reliable and comfortable providing both practical and person-centred support.
The role includes a mixture of personal care, mobility assistance, community access and daily living support, along with accurate documentation and reporting.
Current Shift Times
This position operates across a Monday to Sunday roster, with current shifts including:
- Morning shifts – approximately 8:00am to 10:00am
- Afternoon shifts – approximately 1:00pm to 5:00pm
- Evening shifts – approximately 5:00pm to 8:00pm
Key Responsibilities
- Support participants with personal care and activities of daily living
- Provide hoisting and mobility assistance where required
- Support participants with mental health and psychosocial needs
- Assist with community access, appointments and social activities
- Support participants to attend medical and allied health appointments
- Medication administration
- Domestic assistance and household tasks
- Encourage independence and participation in everyday routines
- Complete detailed shift notes and participant documentation
- Complete ABC reporting and incident reports where required
- Follow participant support plans, routines and organisational procedures
- Communicate effectively with participants, families and the wider support team
About You
We are looking for someone with a calm, mature and professional approach who is comfortable working independently while also communicating effectively with the wider team.
You will need to be confident supporting participants with varying needs and able to maintain professional boundaries while building positive working relationships.
Strong written communication skills are also important, as this role requires regular completion of shift notes, ABC reports and other support documentation.
Previous experience supporting people with mental health, physical disability, personal care or complex support needs will be highly regarded.
Key Requirements
- Previous experience in disability support, aged care, community services or a similar role
- Certificate III or IV in Individual Support, Disability or similar is desirable
- Current NDIS Paid Worker Screening Card, or willingness to obtain
- Current First Aid and CPR certificates, or willingness to obtain
- Current driver's licence
- Reliable and insured vehicle
- Strong written and verbal communication skills
- Ability to work independently and follow established support plans
