Registered Nurse — Community & Aged Care

Hometown Nursing Care is seeking Registered Nurses to provide person-centred nursing care to older people and people with disability across Greater Sydney.
Our office is based in Lakemba. Work locations depend on client needs, your availability and the service area agreed with you.
About the role
You will support clients’ health, independence and dignity while working collaboratively with clients, families, support workers and other health professionals.
Your responsibilities may include:
- Completing nursing assessments and developing, implementing and reviewing individual care plans.
- Providing medication support and administration in accordance with authorised orders, policies and your scope of practice.
- Delivering wound care and other nursing interventions appropriate to your demonstrated competence.
- Monitoring changes in clients’ health, escalating concerns and responding appropriately to deterioration.
- Maintaining accurate clinical notes, progress reports and incident documentation.
- Supporting infection prevention, safe care practices and effective clinical handovers.
- Providing practical education to clients, families and carers.
- Respecting each client’s privacy, consent, cultural preferences and choices.
About you
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia, verifiable through Ahpra.
- Australian working rights.
- Strong assessment, communication and clinical documentation skills.
- Ability to work independently within your competence and collaborate with a wider care team.
- A respectful, reliable and person-centred approach.
- Community nursing, aged-care or disability-sector experience is desirable.
