Bundaleer Home Care is looking for a passionate Clinical Care Manager to lead our nursing team and drive clinical excellence across our growing Home Care service on a 6 month, fill time, fixed term contract with the potential of ongoing employment.

About the role
As our Clinical Care Manager, you'll provide clinical leadership across a diverse Home Care program, ensuring safe, person-centred and evidence-based care for clients receiving Support at Home, DVA Community Nursing, NDIS and icare services.
You'll lead a team of nurses, provide expert clinical guidance, oversee governance and compliance, and work closely with GPs, allied health professionals and care teams to achieve the best outcomes for our clients.
Key Responsibilities
- Provide clinical governance leadership aligned with the Apollo Care Clinical Governance Framework
- Lead clinical incident review, risk mitigation and continuous improvement
- Conduct and coordinate comprehensive clinical assessments
- Develop complex, person‑centred clinical care plans
- Oversee and/or deliver nursing procedures (wound care, medication management, chronic disease support)
- Ensure compliance with:
- Strengthened Aged Care Quality Standards
- DVA Community Nursing Standards
- NDIS Practice Standards (high-intensity supports oversight)
- icare Clinical Framework
- Support clinical components of Support at Home including risk assessments and care plan guidance
- Mentor, coach and support the clinical and non‑clinical workforce
- Build a positive, safety‑first culture with strong clinical leadership
About you
You'll be an experienced Registered Nurse who enjoys leading people and improving clinical outcomes.
You'll bring:
- Current AHPRA registration as a Registered Nurse
- At least 3 years' post-graduate nursing experience
- Experience in aged care, community nursing or disability services (essential)
- Strong clinical assessment and care planning skills
- Leadership, coaching and mentoring experience
- Excellent communication and relationship-building skills
- A passion for delivering exceptional care
