Where you'll be working
Based at Blacktown Hospital, you will join Western Sydney Local Health District (WSLHD) and play a key role in developing and coordinating the Osteoporotic Refracture Prevention Service (ORP).
Working in partnership with endocrinology, hospital clinicians, general practitioners and community providers, you will help strengthen pathways that support patients following minimal trauma fractures and improve access to timely osteoporosis assessment and treatment.
Work across Blacktown and Mount Druitt Hospitals, collaborating with equivalent services across WSLHD.
Be part of a multidisciplinary environment focused on person-centred, evidence-based care.
Build strong partnerships across hospital, primary care, private and community services.
Contribute to a service focused on reducing preventable re-fractures and improving long-term health outcomes.

What you'll be doing
As the Fracture Liaison Coordinator, you will coordinate the ORP service and clinic in collaboration with the endocrinologist, providing specialist clinical support to people identified as being at risk of osteoporosis and re-fracture. You will:
Coordinate and evaluate the ORP, including patient recruitment, assessment, education, treatment pathways and long-term follow-up.
Provide expert clinical advice and develop individualised care management plans for patients with complex health needs.
Empower patients through education and coaching on osteoporosis, re-fracture risk, chronic disease management and self-management.
Lead quality improvement, research, data collection and evaluation activities to strengthen service outcomes and clinical practice.
Develop strong referral pathways and partnerships with healthcare providers to ensure timely access to investigations, treatment and multidisciplinary support.
