Correlate Connections supports older Australians across South West Sydney to maintain their independence, wellbeing, and quality of life while remaining in the comfort of their own homes and communities.
We are currently seeking compassionate, reliable, and dedicated Aged Care Support Workers to join our growing team. This role is ideal for individuals who are passionate about supporting older people to live safely, independently, and with dignity.
As an Aged Care Support Worker, you will play a vital role in enhancing the lives of our clients through person-centred care and meaningful support that promotes choice, respect, and independence.
There may also be opportunities for suitably qualified staff to provide support to people living with disability through our NDIS services.
Key Responsibilities
- Providing person-centred support tailored to individual client needs and goals
- Assisting with daily living activities including meal preparation, light housekeeping, laundry, shopping, and companionship
- Supporting clients with community participation, social engagement, and attending appointments
- Assisting with personal care, including showering, dressing, grooming, toileting, feeding assistance, and mobility support where required
- Providing medication assistance in accordance with organisational policies and training
- Monitoring client wellbeing and reporting any concerns regarding health, safety, or care needs
- Maintaining accurate client records, progress notes, and documentation
- Working collaboratively with clients, families, care coordinators, and healthcare professionals
- Promoting client independence, dignity, and quality of life at all times
- Following care plans and organisational policies to ensure safe and effective service delivery
- Working independently and as part of a supportive team to deliver high-quality care
We Are Looking For Individuals With
- Excellent communication and interpersonal skills
- Strong written English and documentation skills
- A caring, patient, and respectful approach to supporting older people
- Flexibility to work weekdays, weekends, evenings, sleepovers, and overnight shifts if required
- Strong organisational and time-management skills
- Ability to work independently and as part of a team
- A positive attitude and commitment to providing high-quality care
- Understanding of person-centred care principles and the importance of maintaining client dignity, choice, and independence
- Strong attention to detail and a commitment to safe work practices
Essential Criteria
- Certificate III (or higher) in Individual Support (Ageing), Aged Care, Community Services, Disability, Mental Health, Nursing, Allied Health, or a related field (or currently studying)
- Experience supporting older people in aged care, home care, community care, disability support, or a similar setting
- Current First Aid and CPR Certificate
- Current National Police Check
- NSW Working With Children Check (or willingness to obtain)
- Current NSW Driver Licence and reliable vehicle
- Strong understanding of safe manual handling practices
- Ability to assist clients with personal care and mobility support
- Sound administration and computer literacy skills
- Two professional references
Desirable for Staff Interested in Supporting NDIS Participants
For team members who would also like opportunities to work with people living with disability through our NDIS services, the following are highly desirable or can be obtained upon commencement:
- Current NDIS Worker Screening Check
- Completion of the NDIS Worker Orientation Module ("Quality, Safety and You")
- Completion of NDIS Mealtime Management Training (or willingness to undertake training)
