What you'll be doing
As the Clinical Nurse Consultant Home Haemodialysis, you will provide expert clinical leadership and consultancy for patients receiving, or preparing to receive, home haemodialysis. You will work closely with patients, carers and healthcare professionals to deliver high quality, evidence-based care while leading education, service improvement and clinical excellence.
Provide expert clinical advice and specialist consultancy for home haemodialysis patients and their carers
Develop, implement and evaluate individualised care plans for patients with complex health needs
Lead education and support for nursing staff, multidisciplinary teams and patients to promote best practice
Drive quality improvement, research and evidence-based initiatives to enhance patient care and service delivery
Collaborate with internal and external stakeholders to coordinate safe, person-centred care across the renal service

Essential Criteria
Selection Criteria
Minimum of five ( years full time equivalent post registration experience, with at least three ( years current full time equivalent experience in Renal and Dialysis nursing.
Post graduate qualifications relevant to the speciality or other such qualifications deemed appropriate by the public hospital
Demonstrated ability to work independently and within a collaborative team environment.
Demonstrated commitment to quality improvement and improved health care outcomes, with appropriate computer and research skills.
Proven excellent interpersonal, communication and teaching skills
Ability to provide education and counselling for patients and families potentially requiring renal replacement therapy.
