Our client, Charles Disability Services is a registered NDIS disability care provider committed to delivering high-quality, person-centred support services to individuals living with disabilities and mental health challenges. We provide a broad range of services including community participation, in-home support, mental health support, support coordination, accommodation assistance, psychosocial support, and wheelchair transport services.

We are seeking compassionate and dedicated individuals to join our team at Charles Disability Services. As a Community Support Worker, you will play a vital role in supporting people with disabilities and helping them achieve their goals. The ideal candidate is empathetic, reliable, and committed to empowering individuals to live more independent and fulfilling lives.
Roles and Responsibilities: -
- Ensuring effective and efficient delivery of services by coordinating support plans, schedules, and resources, while maintaining a client-focused approach.
- Conducting comprehensive assessments of clients' needs and abilities and developing programs and policies.
- Developing individualized service plans in collaboration with clients and their families.
- Conducting thorough interviews to gain a comprehensive understanding and assess the nature and scope of the challenges faced by the clients.
- Coordinating and facilitating access to a range of support services, including healthcare, welfare, housing, education, employment, and community resources.
- Advocate on behalf of clients to ensure they receive appropriate and timely services.
- Foster partnerships with local organizations, businesses, and community groups to create inclusive opportunities for individuals with disabilities.
- Providing guidance on budgeting and financial management, while assisting clients in accessing financial assistance programs.
- Organizing and collaborating with community groups, welfare agencies, government entities to address community issues.
- Referring clients to relevant agencies or organizations capable of offering additional assistance and support.
- Maintaining accurate and up-to-date client records, and regularly monitoring clients' progress to ensure effectiveness of programs implemented.
- Prepare regular reports on service delivery, outcomes, and challenges.
- Providing guidance and assisting families and delivering educational and caregiving services for children and individuals with disabilities.
- Assisting young people in resolving social, emotional and financial problems to support their overall development.
- Preparing proposals for funding and resources, and compiling reports for government bodies and various agencies.
- Demonstrating cultural sensitivity and competence while working with clients from diverse backgrounds, addressing their unique cultural needs.
Skills and Attributes: -
- Diploma, Advance diploma or Bachelor's degree in a related field.
- Minimum 1-2 years’ experience working in community services or a similar field.
- Knowledge of relevant legislation and policies related to community services.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with clients, families, and a multidisciplinary team.
- Empathy, patience, and a genuine commitment to promoting the rights and well-being of individuals with disabilities.
Job Type & Salary: -
- This is a full-time permanent job.
- The base salary will be AUD 77,000- AUD 87,000 plus superannuation.
Working Hours: -
- Availability to work during evenings, weekends, and holidays as required.
