We are seeking an experienced and motivated Clinical Care Coordinator (CCC) to join our Nursing Management Team and play a key leadership role in delivering high-quality, person-centred care.

About the Role
Reporting to the Clinical Services Manager, the CCC is responsible for coordinating and leading the delivery of clinical care to residents, ensuring compliance with regulatory standards and driving best practice within the Clinical Governance Framework.
Key responsibilities are as follows:
Care Coordination:
- Oversee the development, implementation and evaluation of resident care plans
- Lead clinical assessments and care planning to meet individual resident needs
- Ensure timely, responsive, and high-quality care delivery
- Promote a strong focus on resident outcomes and experience
People Leadership:
- Lead, mentor, and support nursing staff and carers
- Foster a positive, respectful and team-oriented culture
- Support ongoing staff development, capability and performance
- Provide clinical guidance and promote best practice standards
Quality & Clinical Governance:
- Support quality improvement initiatives and accreditation requirements
- Maintain high standards of documentation and clinical compliance
- Contribute to incident management, risk mitigation and continuous improvement
- Monitor clinical outcomes and proactively address emerging risks
Collaboration and Communication:
- Act as a key liaison between residents, families and the clinical team
- Support open communication and transparency in care delivery
- Work closely with multidisciplinary teams to ensure coordinated care.
About You
We are looking for someone who is:
- A Registered Nurse (AHPRA registered)
- Experienced in aged care, ideally in a leadership or senior clinical role
- Confident in clinical assessment, care planning and decision-making
- A strong communicator who can engage staff, residents and families
- Passionate about delivering resident-centred care and leading teams

Job Types: Full-time, Permanent
Pay: $110,000.00 – $140,000.00 per year
