Our Care Connect program is an aftercare service, supporting people experiencing a suicidal crisis or who have attempted suicide. The service focuses on empowering people to connect with supports in their recovery.

What you'll be doing
- Provide non-clinical, psychosocial support to people who have been discharged from hospital after a suicidal crisis or attempt
- Support participants to develop personal safety plans
- Support participants to explore their goals, and engage with other relevant clinical, community and local services
- Support participants with building a strong therapeutic alliance based on their identified needs
What we're looking for
- Diploma or Cert IV (minimum) qualification in Mental Health or Community Services
- At least 2 years’ experience in a suicide prevention /aftercare or mental health setting.
- Experience providing person-centred support, safety planning and case management with people experiencing suicide ideation
