Job Profile:
This position ensures the effective delivery of customer services by coordinating supports to NDIS participants consistent with organisational values and policies. This position will also be responsible for developing, coordinating and facilitating case management support to Non-NDIS participants - who wish to join National Disability Insurance Scheme.

Duty Statement:
- Provide Support Coordination service for NDIS participants
- Provide case management supports for non-NDIS participants.
- Assist NDIS participants with their plan implementation
- Work within funding level associated to each participant
- Facilitate effective working relationships and communications with the carers, family, external services and other key stake holders
- Undertake tasks as directed by the Management team
- Attend all staff meetings and training
- Be an effective planner
- Be organised and proactive and future focussed
Requirements for Appointment:
The successful applicant must have:
- Minimum Certificate IV in Mental Health/ Disability/ Community services
- NDIS Worker Screening Check
- A valid National Police Check (issued within 3-month period) or willing to undergo a National Police Check
- Certificate to confirm that NDIS Worker Orientation is completed
- Current first aid certificate
- A current driver licence and a car with comprehensive car insurance
- Willingness to sign a confidentiality agreement
Selection Criteria:
- A relevant qualification
- An understanding of NDIS and completion certificate to prove that you have completed Worker Orientation Module designed by NDIS Quality and Safeguards commission
- Sound knowledge of National Disability Insurance Scheme and experience as a NDIS Support Coordinator
- Sound knowledge and understanding of Psycho-social disabilities
- NDIS Worker Screening Check (WWVP Card) and a current Police Clearance Certificate (within last three months)
- Proven ability to work individually and as part of a team to motivate, support and communicate effectively with people who are at risk of harm and/or exhibiting challenging behaviors.
- A current driver licence and comprehensive car insurance
