Female Disability Support Worker, Community Access (Hampton Park VIC)

Shifts: Mondays and Wednesdays, 10:00 AM to 1:00 PM

About the Role
Harmony House Community Services is seeking a compassionate and reliable Female Disability Support Worker to provide community access support for a male participant living with mental health challenges in Hampton Park.
This role focuses on promoting independence, social engagement, and wellbeing through meaningful community activities in a supportive and respectful environment.
Key Responsibilities
- Provide one-on-one community access support
- Assist the participant to engage in social and recreational activities
- Promote independence, confidence, and emotional wellbeing
- Build positive and professional relationships
- Maintain accurate progress notes and documentation
- Follow individual support plans and company policies
Requirements
- Female support worker (participant preference)
- Current NDIS Worker Screening Check
- Current Working with Children Check
- Valid First Aid and CPR Certificate
- Australian driver's licence and reliable vehicle
- Comprehensive car insurance
- Excellent communication and interpersonal skills
- Experience supporting individuals with mental health conditions is highly regarded
