Your new role:
In this role, you'll be a vital connector, advocate and health support professional for Aboriginal and Torres Strait Islander people living with chronic disease.
You will work collaboratively across hospital and community services to strengthen culturally safe care and empower individuals and families in their health journey.
Coordinate and support chronic disease care pathways, linking individuals and families with the right health and social services.
Deliver culturally appropriate clinical support, health promotion and chronic disease education to improve long-term health outcomes.
Build strong relationships with community, stakeholders and service providers to identify priority needs and drive meaningful action.
Collect and report activity data, contribute to planning, and participate in key meetings to support continuous improvement in Aboriginal and Torres Strait Islander health.

You thrive in a role where communication, cultural safety and self-direction are essential.
Deep understanding of Aboriginal and Torres Strait Islander cultures, community protocols and the determinants influencing health outcomes.
Strong communication and interpersonal skills, with the ability to engage respectfully and meaningfully with individuals, families and community.
Demonstrated clinical knowledge in chronic disease care (e.g., diabetes, respiratory, cardiac) and a commitment to ongoing learning.
Ability to work independently, manage your workload effectively and contribute to continuous improvement within the service.
