About the Role
Join our team as a Care Coordinator on a full time ongoing basis, to work within the Emergency Department at Monash Medical Centre based in Clayton. Part time applications will be considered.
As the Grade 3 Care Coordinator, you will be an experienced and skilled Social Worker with consolidated psychosocial assessment and planning skills and use evidence to inform practice to enable you to effectively manage complexity in a fast paced Emergency Department. This position will provide support across Casey, Clayton, and Dandenong Emergency Departments, with a flexible roster that includes weekends, as well as early and late shifts (0800-1630 and 1300-2100), offering both direct and remote support. The ability to perform some work from home may be considered on an ad hoc basis.
Key areas of responsibility will include:
- Expert Knowledge Required: Strong understanding of service systems such as Family Violence, Child Protection, My Aged Care, Addiction Medicine, NDIS, Mental Health, and the recovery model, ensuring high-quality, evidence-based service delivery.
- Collaborative Work Environment: Ability to work within a diverse team of Allied Health clinical experts, including physiotherapists, occupational therapists, speech pathologists, social workers, and podiatrists, to provide patient-centred care and achieve exceptional outcomes.
- Transdisciplinary Skill Development: Develop and apply transdisciplinary skills, providing leadership and guidance to the Emergency Department Care Coordination team.
- Clinical Leadership & Care Coordination: Lead discharge planning, care coordination, and liaise with external service providers to enhance patient care during vulnerable times.
About Us
The Monash Health Emergency Department Care Coordinator team is a dynamic service made up of dedicated, highly skilled transdisciplinary Allied Health clinicians. You will be part of a well-integrated leadership team with an emphasis around positive culture and proactive management. This role provides opportunities for growth and development with a focus on excellent patient centred care and team development.
About You
You are an experienced Social Worker with a strong background in Emergency Department and/or acute hospital settings, bringing advanced clinical reasoning, assessment and discharge planning skills. You have experience providing clinical education, supervision and support to staff, and enjoy working collaboratively within multidisciplinary teams to achieve positive patient outcomes.
With excellent communication and interpersonal skills, you are proactive, adaptable and committed to continuous professional growth. You may already have transdisciplinary clinical experience or be keen to further develop your skills within the dynamic Emergency Department environment. Experience in leadership, quality improvement and/or service development will be highly regarded.
