In this role, you will help lead the delivery of specialised oncology pharmacy services to a 35-chair day unit and 32 bed Oncology/haematology inpatient ward, supporting the safe and effective use of therapies through clinical review, aseptic compounding, dispensing and patient counselling. You will work as part of a multidisciplinary team to optimise patient outcomes across all cancer care pathways.
As an experienced Cancer Care Pharmacist, you will provide clinical support within the cancer care team contributing to high-quality, patient-centred at one of Australia’s leading private tertiary hospitals.
You will:

Deliver comprehensive clinical pharmacy services to patients receiving cancer treatment at our oncology/haematology wards and day infusion unit

Provide evidence-based medication advice to the multidisciplinary team

Perform clinical review, medication reconciliation and therapy monitoring

Support safe and accurate aseptic compounding services

Contribute to multidisciplinary meetings, case discussions and cancer care planning

Promote medication safety and quality use of medicines initiatives

Provide mentorship, guidance and support to pharmacy team members

What you’ll bring
MUST:
-
Bachelor of Pharmacy or equivalent with current unconditional Pharmacist Registration with AHPRA
-
Strong clinical judgement and attention to detail
-
Demonstrated experience in oncology pharmacy practice

Excellent communication and ability to work collaboratively within a multidisciplinary team.
DESIRABLE:

ANZCAP accredited
-
Experience in aseptic compounding
-
Experience in using CHARM software
