About the Role
The Care Coordinator oversees the provision of high quality, person-centred care for each consumer through a capabilities approach in admission, assessment, care planning and care delivery and evaluation. This important role is the primary point of contact for families for care services and works with the Care Manager, clinical governance team, other LHG teams and health professionals in assisting consumers to live their best life, their way, each and every day.
Key responsibilities include:

Promote a quality experience for each consumer through effective admission, assessment, care planning and care delivery, and evaluation in accordance with legislative and recognised professional standards

Oversee all aspects of care under the direction of the Care Manager ensuring best practice and compliance with the Aged Care Quality Standards

Primary point of contact for families relating to direct care and/or services, issues including concerns and complaints

In collaboration with the General Manager, Care Manager and Quality and Education Coordinator develop and manage a program of Continuous Improvement for the site

Assist in the development of care staff in care provision through on-going coaching and feedback
About You

Registered Nurse with AHPRA registration

Proven leadership and supervision skills. Ability to bring out the best in others.

Ability to deal with sensitive matters with tact, discretion and appropriate confidentiality

Previous Registered Nurse Aged Care experience

Ability to work collaboratively with a diverse range of people
Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.
