ABOUT US:
Compassionate Hands and Disability Services under Jaubert Holdings Pty Ltd is an Australian-registered private company delivering disability and personal support services under the NDIS framework, including community nursing care, life skills support, and community participation. The company primarily focuses on specialised substitute care for young people aged 13–17, providing a supportive camp-based residential environment offering structured out-of-home care and respite services.
The Role:
We are looking for Community Worker who can work within a dynamic and high performing environment.
Key duties will include but not limited to:
- Assess client needs and provide tailored support in accessing essential services such as health care, housing, employment, and social services.
- Design, coordinate, and assist in implementing community engagement and development initiatives that address social inequalities and promote well-being.
- Work collaboratively with families, health professionals, government agencies, and local stakeholders to deliver integrated and client-centred services.
- Maintain accurate and up-to-date records, reports, and client case notes in accordance with organisational policies, regulatory guidelines, and funding requirements.
- Facilitate referrals to relevant service providers and ensure consistent follow-up to support continuity of care.
- Monitor and evaluate the effectiveness of community programs and interventions, making recommendations for improvements as necessary.
- Promote social inclusion, equity, and respect for cultural and individual diversity in all aspects of service delivery.
- Advocate on behalf of clients to ensure their voices are heard and their rights are upheld within the community and relevant service systems.
- Support clients in developing life skills, increasing independence, and building capacity to engage with their community
Key Requirements:
- Minimum 1 year of experience in relevant field is required
- Minimum Diploma in Aged care, Disability or Community Services.
- First Aid Training certificate
- An understanding of NDIS and a commitment to its principles.
- Service-oriented and able to work both autonomously and collaboratively within a team
- Excellent interpersonal and communication skills
- Ability to work both independently and as part of a multidisciplinary team.
